        Primary care is that care provided by physicians specifically trained for and skilled in comprehensive first contact and continuing care for people with any un-diagnosed sign, symptom, or health concern (the "undifferentiated" patient) not limited by problem origin (biological, behavioral, or social), organ system, or diagnosis.
        Primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses in a variety of health care settings (e.g., office, inpatient, critical care, long-term care, home care, day care, etc.). As part of a medical home, we do provide care coordination across multiple settings, which is defined as:
          "... a patient- and family-centered, team-based activity designed to assess and meet the needs of patients, while helping them navigate effectively and efficiently through the health care system. Clinical coordination involves determining where to send the patient next (e.g., sequencing among specialists), what information about the patient is necessary to transfer among health care entities, and how accountability and responsibility is managed among all health care professionals (doctors, nurses, social workers, care managers, supporting staff, etc.). Care coordination addresses potential gaps in meeting patients' interrelated medical, social, developmental, behavioral, educational, informal support system, and financial needs in order to achieve optimal health, wellness, or end-of-life outcomes, according to patient preferences." - Agency for Health Research and Quality
        Primary care is performed and managed by a personal physician often collaborating with other health professionals, and utilizing consultation or referral as appropriate.
         Primary care provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services. Primary care promotes effective communication with patients and encourages the role of the patient as a partner in health care.
About Dr. Bardia Asgari:
      "I come from a lineage of dedicated physicians, with both my parents forging impactful careers in medicine—my mother as an anesthesiologist and my father as a surgeon in Tehran, Iran. Inspired by their commitment to healthcare, I pursued my own medical education at Carol Davila Medical University in Bucharest, Romania, where I graduated at the age of 24 in 1993. Following my graduation, I honed my skills as a resident in orthopedics at Bucharest Emergency Hospital for four years. In 1997, I transitioned to the United States, where I completed my Internal Medicine residency at St. Vincent Medical Center in Bridgeport, Connecticut, in 2000. After serving as an attending physician until 2007, I embarked on my journey as a solo practitioner at Primary Care of Shelton LLC, where I have been privileged to provide personalized care to my patients since April 2007."
